There has been a dramatic evolution in donor selection for heart and lung transplantation that has challenged traditional matching criteria. A changing donor demographic, DCD transplantation, and organ allocation policy have collectively re-shaped the field’s perspectives on the acceptable donor. Novel risk models integrating donor and recipient characteristics have allowed better prediction of highly relevant short-term and long-term recipient outcomes. In addition, technological advances in organ preservation systems and approaches are enhancing organ function, prolonging ex-vivo times, and hold the potential for graft modification.

In recognition of these innovations, the Journal of Heart and Lung Transplantation (JHLT), an official publication of the International Society for Heart and Lung Transplantation (ISHLT), will release a special issue in Q4 of 2026 dedicated to manuscripts exploring aspects of donor selection and organ preservation related to thoracic transplantation. Submissions for this issue are now being accepted until Tuesday, 30 June 2026.

Please submit your work through the JHLT article submission system. When submitting your manuscript, please state in the cover letter that you would like your paper considered for the Special Issue and select “VSI: Donor Selection” from the “Article Type” pulldown menu.
