Register for this meeting:  Online Registration or use the ISHLT Official Registration Form (PDF).  You do NOT need to register for the Annual Meeting in order to register for the Academy.

Registration for the Academy must be made via the ISHLT website or via the Official ISHLT Registration Form. Online registration is strongly encouraged.
